THIS ALSAFE ANNOUNCES THE RELEASE OF AN UPDATED VERSION OF THE DEPARTMENT OF DEFENSE HUMAN FACTORS ANALYSIS AND CLASSIFICATION SYSTEM (DOD.HFACS). REFERENCES A REQUIRES ALL DEPARTMENT OF DEFENSE (DOD) COMPONENTS TO CLASSIFY AND REPORT HUMAN FACTORS CONTRIBUTIONS TO MISHAPS AND HAZARDS ACCORDING TO A COMMON HUMAN FACTORS TAXONOMY. REFERENCE B ESTABLISHES DOD HFACS AS THAT TAXONOMY. IN ACCORDANCE WITH REFERENCE C, THE SERVICES AGREED TO REPLACE THE CURRENT VERSION OF DOD HFACS WITH DOD HFACS 8.0. 2. WITHIN THE DEPARTMENT OF THE NAVY, INVESTIGATORS WILL REPORT HUMAN FACTORS FOR ALL EVENTS OCCURRING ON OR AFTER 1 APRIL 2023 USING THE DOD HFACS 8.0. THE CHANGE TO DOD HFACS 8.0 WILL OCCUR AUTOMATICALLY WITHIN RISK MANAGEMENT INFORMATION (RMI) SERIOUS INCIDENT REPORTING (SIR). FOR AFFECTED EVENTS, RMI WILL ONLY PROVIDE INVESTIGATORS WITH THE OPTION TO SELECT DOD HFACS 8.0 CODES. ALTHOUGH STRUCTURALLY SIMILAR TO PREVIOUS VERSIONS OF DOD HFACS, THE CODES CONTAINED IN VERSION 8.0 HAVE BEEN UPDATED TO IMPROVE CLARITY, REDUCE EMOTIONALLY LOADED TERMS, AND TO CODE FOR CERTAIN LATENT CAUSES OF UNSAFE ACTS THAT PREVIOUS DOD HFACS VERSIONS WERE UNABLE TO CHARACTERIZE. 3. THE CHANGE IN DOD HFACS 8.0 WILL BE ACCOMPANIED BY A CHANGE IN THE PROCEDURE FOR ENTERING DOD HFACS CODES WITHIN RMI. DOD HFACS CODES WILL BE ENTERED DURING ENTRY OF MISHAP FACTORS. WHEN INVESTIGATORS ENTER A MISHAP FACTOR AND DEEM IT TO BE A HUMAN FACTOR, RMI WILL PROMPT THEM TO ENTER ONE UNSAFE ACT CODE TO DESCRIBE WHAT OCCURRED IN THAT FACTOR AND PRECONDITION, SUPERVISORY, AND ORGANIZATIONAL CODES TO EXPLAIN WHY IT OCCURRED. IF THE MISHAP FACTOR DESCRIBES SUPERVISORY OR ORGANIZATIONAL ISSUES THAT CANNOT BE TIED TO SPECIFIC INDIVIDUALS, RMI WILL PERMIT INVESTIGATORS TO ENTER SUPERVISORY OR ORGANIZATIONAL CODES WITHOUT AN ACCOMPANYING UNSAFE ACT CODE. IF INVESTIGATORS ENTER AN UNSAFE ACT CODE, RMI WILL REQUIRE THEM TO ENTER AT LEAST ONE PRECONDITION CODE TO DESCRIBE WHY THE ACT OCCURRED. 4.
