B $600,000 or more but less than $2,500,000 Permanent partial disability or three or more persons hospitalized as inpatients
C $60,000 or more but less than $600,000 Nonfatal injury resulting in loss of time from work beyond day/shift when injury occurred
D $25,000 or more but less than $60,000 Recordable injury or illness* not otherwise classified as a Class A, B, or C
E $1 or more but less than $24,999 Injury or damage not otherwise classified as a Class A, B, C, or D

The Class D definition was promulgated by DoDI 6055.07 on 06 Jun 2011.

* Recordable Injury or Illness:

  • For civilian personnel, an occupational injury or illness meeting the recording requirements of part 1904 of Reference (h).
  • For military personnel, an on-duty injury or occupational illness meeting the recording requirements of part 1904 of Reference (h) or an off-duty injury resulting in death or 1 or more days away from work.
Reference (h) is OSHA 1904.7 - Recording and Reporting Occupational Injuries and Illness:
  • Any work-related fatality.
  • Any work-related injury or illness that results in loss of consciousness, days away from work, restricted work, or transfer to another job.
  • Any work-related injury or illness requiring medical treatment beyond first aid.
  • Any work-related diagnosed case of cancer, chronic irreversible diseases, fractured or cracked bones or teeth, and punctured eardrums.
  • There are also special recording criteria for work-related cases involving: needlesticks and sharps injuries; medical removal; hearing loss; and tuberculosis.
